On February 15, 2016, Agnes T. Slezak (nee Mierzwinski) beloved wife of late John J. Slezak devoted mother of Dominick Slezak & his wife Patricia, Gregory Slezak & his wife Denise, Theresa Bankard & her late husband Michael, and Joseph Slezak & his wife Kimberly, loving grandmother of Tracy Morehouse, Christopher Slezak, Melissa Wehner, Rachel Lloyd, Kevin Slezak, Kelly Raffensberger, and Hope Slezak. Agnes is also survived by 8 great-grandchildren and other loving relatives and friends.
